Thank you for taking the necessary time to complete all forms before your visit. These forms are needed for the doctor's thorough analysis and will also save you time during your visit! If you are unable to complete your paperwork before your visit, please arrive 20 minutes before your appointment time to ensure they can be completed
Our elegant office is located in the lobby of the CityWestPlace building #4 but the physical address may not match the driving directions provided by your GPS
If you are traveling from outside CityWestPlace campus please note that you will enter the visitor parking garage located on Private Drive and closest to the Beltway 8 feeder. Upon arrival at the parking garage you will need to check in with security, so please have your driver's license ready to present. There is convenient visitor parking near our office entrance. Make your way around the corner into Building 4. We are at the first door on the right upon entering the building from the parking garage.
